This particular certificate was awarded to James W. Mulvey and made available for this commemorative presentation by his daughter, Warden Kathleen Mulvey. The wording and names above, including that of Warden Mulvey's father, James W. Mulvey, constitute Page 1 of the order. "Page 2" describes the crash, the action taken, and praises received by the Department for that action. The order concludes:

Because of the evidence of unusual devotion to duty displayed by the above mentioned members in this incident, the Departmental Meritorious Service Award is bestowed upon them in accordance with Rule 3.57 of the Rules and Regulations.
